Evidence Pack Builder

A free tool to plan the evidence for a benefits claim or appeal. It lays out a request letter for each person you want to ask, plus a checklist and diary templates. It is general information, not legal advice, and it cannot tell you how a decision will go.

Why evidence matters

A letter from your GP, consultant or support worker describing how your condition affects daily living and mobility gives a decision maker something concrete to work from.

What Evidence to Gather

  • GP letter: How your condition affects daily living and mobility
  • Consultant reports: Specialist assessments and treatment plans
  • Support worker statements: Observations of your daily difficulties
  • Care records: Details of any personal care or support you receive
  • Medication list: All prescribed medications and their side effects

How the tool helps

You answer a few questions about your situation and who you want to ask. The builder lays out a letter for each of them naming the activities you need them to comment on, and gives you a checklist and diary templates to go with it. Everything you type stays in your own browser.
